Referral Form – Finding and Completing

The Single Service Request form (HCTP 0037) replaces referral form CSF 3155. This form is used by schools when making referrals to Children's Services. The form is used for a variety of referrals including child protection. Forms and Guidance can be downloaded from:

The Designated Senior Person should keep a copy of the referral.

When making a referral to Children's Services it is essential that all relevant information is included. The referral form is itself a useful guide as to relevant information.
